Records Release
Your medical records are available upon request. When you find a new provider, we can provide the with records upon your completion of the HIPAA Authorization Form. In order to release your records to your new physician, a signed copy of the release form must be sent to Dr. Jeanne Isaacson. Her fax number is (253) 276-4597. Please be sure to include the Fax number of the provider for whom you would like us to send your records.
